Which Documentation Is Required in Order to Apply for a Premium Extended Validation (EV) Certificate?

Depending on the requesting corporation, a combination of the following documents must be submitted in order to apply for a Premium Extended Validation (EV) SSL certificate:
Required by all:
- Signed Certificate Request. Signed by Certificate Requestor, Certificate Approver.
- Legal Opinion or Accountant Letter. The Registration Authority will notify you during the process to identify which type of document is required based upon the length of time in business and control of the domain listed in certificate request.
- Required for first certificate requested by a given organization, as well as any subsequent change of authorized signer:
Subscriber Agreement. - Notarized Personal Statement Attestation.
- Notarized Face-to-face Attestation Statement.
- Notarized copy of the photo ID used for identity validation.
